Information you choose to share

The consultation form asks for your name, email address, the service you need, a project description, and your preferred contact method. Business name and phone number are optional unless you ask to be contacted by phone or text.

What happens to your request

Your submission is stored with this website hosting service and copied to an AP DeZines Google Sheet in Google Workspace. AP DeZines receives an email notice at info@apdezines.com when the sheet is updated. The notice includes a link to the sheet and the total request count, but not your project description. Authorized AP DeZines users review requests to respond to you and discuss the scope of services.

Submitting this form does not enroll you in a marketing mailing list, take a payment, or automatically reserve an appointment. Any project work or appointment is confirmed separately.

Keep sensitive information out of the form

Do not send passwords, Social Security numbers, tax IDs, account numbers, payment details, or confidential client information. If a project later requires sensitive material, discuss a suitable way to share it before sending it.

Technical processing

The form sends your request to this website over an encrypted connection. The hosting service may process technical information needed to operate the site. The form code does not store your written project details in browser storage.
